The Chick Hatchery Loss Reporting Form

While we do not anticipate a loss, The Chick Hatchery does cover losses of birds that arrive deceased or birds that pass within 24 hours of arrival when properly cared for. Losses must be reported within 24 hours of arrival. Please review proper brooding here: Poultry Care

Issue a refund for Dead Chicks or Shorted Orders. (Shipping costs & Vaccinations are Non-refundable)
Replacements Requested. Replacement orders will be charged a $15.00 per box USPS FEE. (Availability & Minimum Order Requirements Apply)

Reimbursements are limited to the cost of the birds and will NOT include shipping or vaccination service costs. Shipping costs/fees may be refundable through the USPS. You must file a claim with USPS.

Brooder Heat Source & Temperature *

I am using a Heat Lamp. (This is the recommended heat source for shipped chicks)
I am using an Alternate Heast Source.
Brooder maintains a temp of 100 - 105 degrees for the the first 48 hours.

Brooder temperatures are the main cause of chick loss.
