The Chick Hatchery Loss Reporting Form

While we do not anticipate a loss, The Chick Hatchery does cover losses of birds that arrive deceased or birds that pass within 24 hours of arrival when properly cared for. Losses must be reported within 24 hours of arrival. Please review proper brooding here: Poultry Care

Only submit one loss report per order please!
Reimbursements are limited to the cost of the birds and will NOT include shipping or vaccination service costs. Shipping costs/fees may be refundable through the USPS.
We often add extra chicks to help offet any possible loss so please count them.
Brooder temperatures are the main cause of chick loss.
